I would say "Kenny Dies", since he uses Kenny's illness as an excuse for stem cell research, and then used the stem cell's not for Kenny, but to make another Shakeys.
Other episodes where he treats him Badly are "Quest For Ratings", "The Poor Kid", "Best Friend's Forever" and "I Should Have Never Gone Ziplining", but those instances aren't as bad as "Kenny Dies".

Let's not forget about "A Ladder to Heaven" and the way Cartman treated Kenny's "chocolate milk mix," albeit without realizing what exactly he was doing. Then "Poor and Stupid" springs to mind, where Cartman nearly got Kenny killed, albeit in self-defense. Sort of.
That being said, I'd rather go with "Lice Capades" where Cartman does his utmost to frame Kenny for spreading lies. Eh, lice.
